Primary Care Services

Our providers work with patients and your facility’s staff to conduct health assessments and develop personalized, specialized care plans for your residents. They practice independently and in collaboration with your facility’s medical professionals to manage patients’ health needs. They are also trained in preventive medicine and guiding patients to stay healthy for the long haul.

Our nurse practitioners provide a full range of primary care services, including:

  • Performing physical exams and patient observations

  • Recording patients’ medical histories and symptoms

  • Diagnosis and management of acute, chronic, and complex health issues

  • Ordering, administering, and reading diagnostic tests

  • Monitoring and operating medical equipment

  • Prescribing medicine and other non-medicinal treatments and therapies

  • Detecting changes in patients’ health and suggesting modifications to treatment plans

  • Training staff, patients, and family members to manage and prevent illness and injury

  • Educating patients on disease prevention and making healthy lifestyle choices


 
 

Specialty Services

In addition to primary care services, we have nurse practitioners with specialty training to diagnose and treat more complex issues:

 
  • Our psychiatric nurse practitioners are trained to assess and treat a variety of mental health challenges. They can provide diagnoses and prescribe medications, and are trained in gradual dose reduction techniques to assist facilities in complying with health department regulations. They can perform patient wellness checks and offer therapy and counseling sessions.

  • Our wound care-certified nurse practitioners can measure, treat, and manage treatment of all types of wounds, pressure ulcers, tubes and fistulas, and surgical incisions. They are trained to perform biopsies, conduct general skin checks, and teach wound prevention and healing.

  • Our GI nurse practitioners can diagnose and treat issues related to the digestive tract, such as acid reflux, GERD, and other gastrointestinal concerns. They are also trained to perform G-tube insertion and replacement.

  • Our cardiac care nurse practitioners are trained to monitor and assess heart health through the administration of EKGs, echocardiograms, stress tests, and more. We can provide any testing equipment required to the facility, reducing the need for off-site visits.

  • Our urology certified nurse practitioners can provide a range of consultative, diagnostic and treatment services for genitourinary tract issues, including bladder problems, urinary tract infections (UTIs), bladder and kidney cancer, kidney blockage, and kidney stones.
