We provide nurse practitioners for patient care services, including specialized services, and a fully-staffed support team to handle all of the back-office and administrative work to keep things running smoothly and your facility’s staff uninterrupted in their work.

  • There is no fee! We bill patients’ insurance directly.

  • Nurse practitioners receive unique education and training with specialized knowledge and clinical competency to practice in primary care, acute care, and long-term care settings. Nurse practitioners offer more education, autonomy, and experience than an RN, but with clinical expertise. They’re able to prescribe medications, diagnose conditions, and perform many other tasks a doctor can do, but are often more cost-effective.

    Our specially trained geriatric care nurse practitioners blend clinical expertise in diagnosing and treating health conditions with an added focus on disease prevention and health management.

  • They are adept in using all of the various note-taking apps and can adapt to take notes in your current system.

  • We will work with your Medical Directors, Director of Nursing, and Assistant Director of Nursing to set up the visits before any of our nurse practitioners step foot inside your facility. The process is smooth and designed to be turnkey for the provider as well as medical personnel on the floor.

  • We have a billing department that is in constant, direct communication with the nurse practitioners and will receive all necessary documentation to handle patient billing.

  • Each and every patient will get the specific specialized care that is needed in order to provide the best quality of life and care for that patient. There is no minimum number of patients required to be seen in order for us to send in clinicians as needed.

  • Yes. Our wound care certified nurse practitioners are experienced in complete wound care and have a consistently impressive track record of monitoring, tracking, recording, and healing all scopes of wounds.

  • Our providers work well independently but are also educated on all of the laws, rules, and regulations of each state for skilled nursing facilities and are able to work alongside your existing team to assist in any way that you need. SCG also partners with a network of consultative physicians who collaborate with our NPs to confirm their notes and provide additional support.
